Italeri, Academy/MRC and AFV Club offer a 1/35 AH-1W.
The Italeri and Academy boxing are the same moulding, but I can't comment on the AFV offering.

No idea.
I never heard about this outfit! Great Models Webstore probably have them, just like Squadron.
You can always ask!

I'm in the process of finishing up a 1/35 Academy/MRC AH-1W right now. From what I understand, MRC made the original kit, which is reboxed by both Academy and Italeri. For some reason, the Italeri goes for $10 more than the Academy even though they're the same kit, minus the included decals I think.
It's a nice kit, easy build. I'm really enjoying this one.
I also picked up aftermarket decals from Two Bobs called Whiskeys In The Mix. Nice set, gets you decals for 8 different USMC Cobras.

I believe all of them are the same basic kit made by MRC. I have the AH-1T+ Super Gold Cobra. They are relatively inexpensive and you can find them on eBay for not a whole lot of money. I got mine from Toys R Us when they were clearing out model kits for a whopping $4.90. Although they are great kits, they are still dirt cheap.
AH-1W for $10
AH-1W for $8
AH-1W for $5
AH-1T+ Gold Cobra for $4
Not connected with any of the sellers.

In addition to Sabot's links , here are a couple more:
Italeri AH-1W Super Cobra
AH-1W Super Cobra
AH-1T+ Gold Cobra
A little more expensive though.
